Alexander Youth Network is seeking a Case Manager to work in our Outpatient Plus (OPT Plus) program. Qualified applicants should be meet NC requirements for QMHP. QMHP should have either a Bachelors in a Human Services related field (i.e. Psychology, Social Work) and 2 years experience with children/adolescents or a Masters in a Human Services related field and 1 year experience with children/adolescents or Bachelors in a non- Human Services related field and 4 years experience with children/adolescents.
OPT Plus is a combination of best practice outpatient therapy services, monitoring, support, and management of care interventions to be provided for individuals with complex clinical needs that traditional outpatient cannot adequately address. OPT Plus is a level of care between outpatient and intensive in-home services.
OPT Plus is home and community-based treatment service focused on decreasing psychiatric and behavioral symptoms in order to reduce the need for higher levels of care or increase the likelihood of a successful transition to outpatient therapy from higher levels of care.
