Program Mentor PT- Journey Place - Charlotte, NC

Interested in getting hands-on experience in the human services field? Journey Place is seeking qualified professionals to serve in our Transitional Living home as a Program Mentor. This is a part-time, weekend position. Shifts will be Friday - Sunday, 5PM to 10PM.

Pay rate is $11.50/hour

High School Diploma/ Equivalent Required. Experience with population preferred.

SUMMARY

Responsible for coordinating appropriate intervention services for youth. Responsible for maintaining the residence and clients in a clean and safe environment. Assist young adults utilizing skill building activities that are within 5 transition domains: Education, Employment, Health and safety, Community Life Functioning, and Safe and stable living situation/housing.

KEY PERFORMANCE INDICATORS (KPIs)

Complete all necessary paperwork and documentation, including service plans, client assessments and verbal/written summary of each resident at shift change.

All forms of required documentation are completed by the end of the shift and error-free

TR, Inc. direct care staff provide quality services in accordance with Love and Logic, TCI regulations, Joint Commission standards and agency policies

100% of services will meet standards and policies

TR, Inc. direct care staff will demonstrate the ability to function safely, professionally and ethically without the benefit of direct supervision.  This includes serving as resident advocate, ensuring the cleanliness and sanitary condition of residence, providing referrals, facilitating individual and group sessions, and case management (as needed).

100% compliance as evidenced by work product quality, quantity, timeliness and by any client/staff reports.

Establish a supportive and helpful relationship with the youth in transition and other identified team and community members.

Apparent high levels of advocacy and supportive behavior towards residents and other partners as evidenced by scoring minimum average of 3 (meeting expectations) via satisfaction survey

Continuously assure quality improvement focus for Program and the agency as a whole.

Reports quality issues or problems that impact the residents and implements process improvement strategies as directed

Complete all necessary paperwork and documentation, including service plans, client assessments and verbal/written summary of each resident at shift change.

Confidentially provide updated information to other agencies on client status as required.  Act as an advocate for client's needs as needed with appropriate parties (i.e. schools, other agencies, court.)

100% compliance of HIPPA regulations.  Staff assures resident information is provided as appropriate and follows all federal, state, and agency guidelines as it relations to privacy and confidentiality.

Plan and implement recreational and educational activities for clients. (i.e. life skill training, group activities, house meetings)

100% of the time staff attempts to engage with each resident during shift as evidenced by shift change over and other documentation.

Oversee administration of doctor-referred medication to residents as prescribed and document in client's charts.

100% compliance as evidenced by completion of documentation and medication administration, including MAR transcription, pill count (if applicable) and distribution of medication.

Participate in required meetings including but not limited to serving on one agency committee, staff meetings, and mandated and/or optional education.

100% compliance as evidenced by attendance at mandatory staff meetings.

Ensure safe and confidential environment for clients of program.  This includes applying rules and expectations consistently and ensuring Program Management is advised of needed assistance in regards to escalations appropriately. 

Staff proactively responds to situations that arise during shift 100% of the time and appropriately requests supervisory intervention.

Completes all mandated training

100% of required training will be completed within specified time frame

Attendance:  Has minimal occurrences of unscheduled absences or lateness

No more than 5 unscheduled events per year

EDUCATION and/or EXPERIENCE

High school diploma is required.  B.A. /B.S. in a related human service field preferred. 6 months Mental Health experience is preferred, but not required.

Valid  NC or SC Driver's license.
