Therapeutic Foster Care Treatment Coach - Rutherfordton, NC

Interested in an opportunity with a leading mental health organization in the Rutherfordton County area?  Alexander Youth Network is looking for a Qualified Professional for an opportunity as a Treatment Coach in our Therapeutic Foster Care department.

Must be a Qualified Mental Health Professional (Masters in Human Services 1 year experience with population or Bachelors in Human Services 2 years experience with population or Bachelors in Human Services 4 years experience with population or provisional/ full licensure).

Salary is $40,000 annually.

SUMMARY

Manages all supervision and support services for foster parents; performs case management tasks for children placed in foster care; participates in pre-service and in-service training in order to provide families with adequate training; maintains flexible work schedule to include on-call coverage 24/7/365; approves and schedules contact between foster parents and all internal/external parties according to each child's individual case plan; maintains foster parent files; schedules, communicates, and coordinates multi-disciplinary case plan reviews, including Child and Family Team meetings.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

ESSENTIAL DUTIES AND RESPONSIBILITES

KEY PERFORMANCE INDICATORS (KPIs)

Manages foster family homes through direct supervision, home visits, phone consultation, crisis intervention, and community-based meetings

100% of treatment parents have met their KPIs

Completes home visits to ensure appropriate supervision

100% of weekly supervision visits are conducted and documented

Maintains active service plan on each client

100% of events provided and billed are on active service plan

Completes documentation including reviewing progress notes, assessments, case plan reviews, court summaries, and discharge summaries

80% of clients go to a discharge placement which matches the discharge plan

Coordinates and links clients to appropriate services and initiates discharge planning starting at intake for each client.

Will meet recovery goals, 90% of the time through change of level of care within the average length of stay (6-9 months)

Completes daily billing by 10:00am every week day.

100% of activity and verification reports are completed same day of service

Accurately maintains client Electronic Health Record

Data entered in EHR and CDT is accurate 99% of the time

Completes all mandated training

100% of required training will be completed within specified time frame

Successfully meets discharge outcomes

90% of the time, will meet discharge plan outcomes

Attendance:  Has minimal occurrences of unscheduled absences or lateness

No more than 3 unscheduled events per year

SUPERVISORY RESPONSIBILITIES

Directly supervises all foster family members.  Supervision is carried out in accordance with the agency's policies, procedures and applicable regulations and laws

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

Must be Qualified Mental Health Professional. B.A or B.S. in Social Work or a related human service field preferred. Two to three years' experience in foster care and one year in behavioral healthcare preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

Valid NC or SC Driver's License required.  QMHP required.
