Transition Coordinator - PRTF - Charlotte, NC

Under direct supervision of the Director of Access Services, the transition coordinator is responsible for providing case management to those children that have mental health challenges gain access to appropriate mental health services and other community services. Their duties include meeting with clients and their families, connecting clients with professional services and maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed.

Key Duties

Accessing client needs, and developing strategies to help the client. Utilizing the Child Family Team process as required. Utilizing weekly or monthly supervision with supervisor.

Providing support and resources for clients; good working relationship with support networks, government resources, and community resources.

Evaluating clients' progress periodically and making adjustments as needed to improve outcomes by entering NCTOPPS, IRIS and utilizing the Acorn tool as required.

Advocating and raising awareness on behalf of clients and the needs of the community and local services through excellent Case Management.

Bachelor's Degree required, Must be QMHP as verified by AYN, Behavioral health front desk, medical office experience preferred. Proficiency in MS Office applications.
