SUMMARY Supports and implements the directives and priorities set forth by the Executive Director and the Senior Management Team. Ensures that a supervisor is available at all times between 8:00 am and 6:00 pm, and “first responder” coverage is available 24 hours a day, 365 days a year. The Intensive In-home Coordinator will assist in the day-to-day coordination of the Intensive In-Home program and will provide brief behavioral and emotional services to children and their families within a community-based model. The coordinator is a member of a 3-person team, with a maximum case load of 12 families in providing such services; conducts skill-building activities, provides parenting education, intervenes during a crisis, provides on-call coverage within the intensive in-home team 24/7/365. Responds appropriately and timely to crisis situations; develops, reviews, and follows individual Person Centered Plans with the Child and Family Teams for each child on their caseload. Provides psycho education to Child and Family Team members; facilitates effective step-down to less restrictive services and increased reliance on natural supports over time. Collaborates, coordinates and advocates with collateral agencies and natural supports in order to meet family and consumer needs in all domains. Services are provided in the client’s home, school, and community settings, under the direction of the Intensive In-home Therapist.

SUMMARY Manages all supervision and support services for foster parents; performs case management tasks for children placed in foster care; participates in pre-service and in-service training in order to provide families with adequate training; maintains flexible work schedule to include on-call coverage 24/7/365; approves and schedules contact between foster parents and all internal/external parties according to each child's individual case plan; maintains foster parent files; schedules, communicates, and coordinates multi-disciplinary case plan reviews, including Child and Family Team meetings. The RN staff to promote consistency and reliability of care, assists in the development of policies and procedures, assists in ensuring compliance with Joint Commission and other regulatory standards. The Registered Nurse will monitor the milieu and all restrictive interventions to ensure the safety and welfare of FBC clients. Alexander Youth Network/Youth Focus is seeking qualified applicants to serve in our Transitional Living Program in Greensboro, NC. Here’s some information about our HEARTH Transitional Living Program: The HEARTH Transitional Living Program provides supervised housing for male and female youth ages 18-21 for up to 18 months. During their stay in the program, all of a youth’s living expenses are taken care of so they can focus on accomplishing goals that will ensure success in their future. Each youth sets goals specific to their situation to transition from the program to stable housing, obtain their high school diploma and/or college degree, receive needed vocational training to gain and maintain employment, develop permanent connections with caring adults, and improve their social-emotional well-being. During their stay in the program, they gain life skills such as conflict resolution, money management, meal planning and preparation, goal setting, and more. This program is designed to be similar to dormitory-style housing with clustered apartments leased by Youth Focus. Two youth share a two-bedroom apartment, and one apartment serves as the staff office and a common gathering space for program participants. Located in High Point, NC, this program allows youth some independence while still having staff support nearby as they learn and practice independent living skills. Youth who have benefited greatly from this program include homeless youth and young people who have aged out of the foster care system without the needed support to live successfully on their own. This position Part Time position. Must be able to attend a two week orientation.

This is a PRN (as needed) position. The Youth Focus' My Sister Susan's House program provides housing and transitional living services for pregnant and parenting youth ages 16-21 for up to 18 months. The program coach provides support to youth in a residential setting, helping them learn independent living skills. The program coach is responsible for assisting clients with goal setting, providing case management, facilitating groups, supervising outings, providing transportation, and providing overall support in a therapeutic environment. This position would be good for someone interested in social work, human development, child development, coaching, psychology, non-profit, direct care, mentoring, counseling, human services, sociology, criminal justice, juvenile criminal prevention services, homelessness, housing, LGBTQ issues, teen pregnancy, marginalized and underserved populations, mental health and runaway and homeless youth. Alexander Youth Network is looking for part-time, split shift morning and afternoon drivers to transport children to and from our Charlotte Day Treatment program. Day Treatment is an alternative program for children who have social and behavioral problems that create challenges for them in traditional classroom settings. Qualified applicants must be willing to safely transport children with emotional and behavioral issues. You must possess a valid NC or SC driver's license and have a clean driving record with no suspensions in the last five years. Experience operating a 15-passenger van is strongly preferred. Experience working with children preferred. The position is part-time (6-8am, 2-4pm), 20 hours weekly. A CDL is not required. Must be able to attend orientation and DOT training from Mon-Fri, 9am-5pm. Pay rate is $17.75/hour.
