Job Title:  Transition Specialist - Outreach

SUMMARY

The person in this role will coordinate the services for young adults in transition. This may include working with various agencies, providers, faith based organizations, and individual partnerships. They will develop, implement, and manage the individualized plans for young adults ages 16-24. They will help to ensure young adults are provided with the tools, support, and resources necessary to become self sufficient.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Maintain client related data systems, including case notes and complete electric health entries.
  • Maintain accurate and up to date case documentation
  • Establish a supportive and helpful relationship with the young adult in transition and other identified team and community members.
  • Confidentially provide updated information to other agencies on client status as required.  Act as an advocate for client's needs as needed with appropriate parties (i.e. schools, other agencies, court.) 
  • Monitor, link, and coordinate services within 5 transition domains: Education, Employment, Health and safety, Community Life Functioning, and Safe and stable living situation/housing.
  • Completes all mandated training. Demonstrate knowledge of and comply with all agency policies and procedures
  • Attendance:  Has minimal occurrences of unscheduled absences or lateness
  • KEY PERFORMANCE INDICATORS (KPIs)
  • 100% compliance with documentation related to data systems
  • All forms of required documentation are completed by the end of the shift and error-free.
  • Apparent high levels of advocacy and supportive behavior towards residents and other partners as evidenced by scoring minimum average of 3 (meeting expectations) via satisfaction survey
  • 100% compliance of HIPPA regulations.  Staff assures client information is provided as appropriate and follows all federal, state, and agency guidelines as it relations to privacy and confidentiality.
  • 100% of services will meet standards and policies
  • 100% of required training will be completed within specified time frame.
  • No more than 5 unscheduled events per year

QUALIFICATIONS

EDUCATION and/or EXPERIENCE 

B.A. /B.S. in a related human service field required. Experience working with individuals in the mental health population required.  Experience working with youth in transition required.

REQUIRED COMPETENCIES  

Competency is defined as the techniques, procedures, technology, skills or equipment needed to do one's job.  The following competencies are required of this position:  age specific competency for children and adolescents, competency in the Administration of Medications, Therapeutic Crisis Intervention, Life Saving/Safety to include First Aid and CPR; Working with Mental Health Population through utilization of TCI and NMT, Blood Borne Pathogens and Infection Control.
